From Latin labi, “fall”, “slip”.

Lapses are the expression of mental fatigue, short-term memory loss: forgetting to send a document or skipping a step in a procedure. Hyperactivity causes errors like omission, inversion and repetition.

Lapses are not the result of incompetence, but rather an act of forgetfulness that may be committed by highly qualified personnel.
The source of forgetfulness is stress. The problem is not stress itself, but how people manage it mentally. Whenever the ability to manage stress is reduced, the greater the likelihood of lapse-type errors or that inadequate strategies are activated for coping with stressful situations.
